Do not submit detailed medical information
The public form is not intended for diagnoses, detailed medical histories, insurance policy numbers, insurance-card images or payment-card data.
How information is used
Information may be used to respond to requests, communicate about appointments, provide requested services, prepare eligible receipts and keep necessary business records.
Optional marketing
Optional promotional consent is separate from appointment communications and may be withdrawn.
Service providers
The site may use Cloudflare to deliver and protect the website and an email delivery provider such as Resend to transmit appointment requests and confirmations.
Retention and protection
Personal information should be retained only as long as reasonably required for its purpose, business-record needs or applicable legal requirements, and protected using reasonable safeguards.
